Description: Supports the Education & Career Programs and Services Coordinator in the vision of improving positive student outcomes by providing a well written Indicator 13 compliant transition plan. The Transition Consultant will prepare students to transition from high school to adult life and reach positive post school outcomes; will provide technical assistance, trainings, and consultations to special education teacher and other relevant staff, as well as, to various agency personnel; is responsible for the development and coordination of post-school transition planning for students assigned to a caseload; partners with schools and community to support capacity building so students will graduate from high schools with the skills necessary to succeed in global society.
Requirements: Master's Degree from and accredited college or university AND Certified Rehabilitation Counselor Certificate or Rehabilitation Administration OR LBS-I with coursework or experience in transitional activities OR LBS-II credential preferred.
