Case Manager

  • ·         Work in collaboration with special education staff, administrators and other district staff members to ensure the best use of resources in providing services for students
  • ·         Serve as a resource to school personnel on the nature, causes, and solutions to the learning difficulties of students
  • ·         Assist teachers with alignment,  program development and coordination of services in order for students to reach established IEP goals
  • ·         Assist and provide support with complex individualized education plans including facilitating meeting and advising principals for more involved IEP process
  • ·         Work to coordinate services for IDEA eligible students within the school, across the district, and throughout the community
  • ·         Contact outside agencies to provide additional services as requested by the IEP team
  • ·         Ensures students participate in the appropriate statewide assessment and that the teachers of the students provide appropriate accommodations on those assessments
  • ·         Ensure teachers, test coordinators and administrators are abreast of the regulations concerning appropriate accommodations on statewide assessments
  • ·         Performs observations of students, review of student's educational history, an d evaluation and analysis of the student's performance when requested
  • ·         Actively participates in the deliberations and classification of educationally disabled students for both initial evaluations and re-evaluations
  • ·         Facilitates re-evaluation process, documentation and data collection
  • ·         Gathers data necessary for initial evaluations and re-evaluations as requested by the psychometrist
  • ·         Ensures initial evaluation and re-evaluations are completed within required timeframes
  • ·         Documents student eligibility in logs
  • ·         Creates and maintains student eligibility files
  • ·         Transfers IEPs from school to school
  • ·         Research student files for other educational agencies as requested
  • ·         Requests records from other educational agencies when necessary
  • ·         Assists in providing staff development for special education teachers, paraprofessionals and administrators related to special education and related services
  • ·         Assists in training new staff members
  • ·         Monitors SEAS, MSIS and student management systems and data
  • ·         Coordinates SEAS
  • ·         May serve as the LEA at IEP meetings when designated by supervisor
  • ·         Conducts regular school department meetings
  • ·         Reviews IEPs for enrolling special education students to determine appropriate placement
  • ·         Reviews courses/scheduling for special education program implementation
  • ·         Monitors student performance and IEPs for appropriate modifications or accommodations
  • ·         Maintains ongoing relationships with parents, students, administrators, special education staff, and related service providers through phone contact, email and individual meetings
  • ·         Responds to acute, unexpected emergency situations
  • ·         Monitors discipline data for students with disabilities to ensure procedures are being followed at the school level regarding students with disabilities
  • ·         Provide thorough an d timely reports, data and information related to students with disabilities as requested to district administrators
  • ·         Facilitates Manifestation Determination IEP meetings when appropriate
  • ·         Informs families of the parental rights and Procedural Safeguards
  • ·         Coordinates transportation for students where that service is listed on the IEP
  • ·         Order and distribute instructional supplies for teachers
