JOB GOAL
:
To coordinate a school health program
including the delivery of services to students and act as a
health resource to staff members in order to enhance health
and wellness in the school community.
Duties are performed in accordance with
standards of the Illinois Nurse Practice Act, Illinois State
Board of Education School Nurse Standards of Care and Chicago
Public Schools District 299 policies and procedures.
PERFORMANCE
RESPONSIBILITIES
Coordination of Nursing
Care
-
Provides direct professional
nursing services, first aid, illness, and emergency care to
students including nursing assessment, identifying health
problems, making referrals for diagnosis and treatment,
recommending educational modifications and accommodations,
providing follow-up and evaluation.

Maintains appropriate
documentation with progress note uploads at least every 7
days using existing CPS technology.

In accordance with Illinois
Nurse Practice Act 225 ILCS 65/50-75, delegates
nursing activities
to other Certified School Nurses, Health
Service Nurses, or licensed practical nurses. "Nursing
activities" require
nursing knowledge, judgment or decision-making.

In accordance with Illinois
Nurse Practice Act 225 ILCS 65/50-75, delegates tasks to
unlicensed school personnel.
"Task" means work not requiring nursing knowledge, judgment
or decision-making.

Provides counseling and
follow-up services to students with disabilities as
delineated in the IEP/504.
-
Follows up on homebound students
needing related health care services.
-
Provides intervention in times
of school crisis, e.g., death, suicide, traumatic
events.
-
Initiates, encourages, and
participates in health education programs for
students.
-
Assists special education
students to accept or adjust to physical, emotional, or
health limitations.
-
Provides health counseling to
parents and students.
-
Provides individual and group
health counseling to special education students.
-
Develops medication protocol to
safely store, administer, document, and monitor the
effectiveness of medication given at school.

Coordinates activities of local
schools concerned with communicable disease regulations.
·
Provide follow-up for deficits in
vision, hearing, growth and development, and other physical
impairments.
Teaching
·
Assists in the orientation of
licensed nurses new to Chicago Public Schools
·
Provides mandatory staff in-service
programs on health topics including Food Allergies and
Diabetes
·
Provides health maintenance and
disease prevention classroom instruction.
Communication
·
Presents and interprets health and
developmental status of students at eligibility
conferences.

Facilitates communication
between administrators, teachers, other school personnel, and
parents/guardians to enhance cooperative action, which will
meet the health and safety needs of students.

Acts as a liaison among the
home, school, community health agencies and the private
medical sector to enhance the health and wellness of the
school community.

Provides health services,
information, and counseling in an effective and positive
manner to enhance the health and wellness of the school
community.

Documents IEP/504 services at
least every seven days.
-
Monitors nursing services
provided by LPN and/or Agency nurses and approves progress
notes via SSM or Easy Trac.
-
Participates in maintaining
accurate medical records to assure compliance with state
mandates including immunizations, physical examinations, and
medical conditions, and the related archival
responsibilities.

Maintains confidentiality
regarding all school and health-related issues.

Documents encounters with staff
regarding consultative student services.

Collaborates with administration and
school personnel to implement Allergy Drill
·
Participates as an active member of
the school community, representing health and wellness.
·
Provides health-related classroom
instruction
.
Organization
-
Manages school health records in
accordance with Illinois State Board of Education School
Records Act and Chicago Public Schools District 299 School
records -Maintenance and Retention of School Student
Records.
-
Participates in maintaining
accurate medical records to assure compliance with state
mandates including immunizations, physical examinations,
vision examinations, and dental examinations and the related
archival responsibilities.
-
Utilizes existing technology
effectively in the performance of duties.
-
Demonstrate effective time
management in performing responsibilities.
Special Education
-
Participates as a member of the
special education team in the identification, assessment
planning, eligibility determination, and creation of the
individual education plan.
-
Completes evaluation to
determine eligibility and/or placement of students into
special education.
-
Recommends and develops goals,
accommodations and modifications of the school program for
students who have a medical condition that impacts access to
their educational program.
-
Develops and maintains current
health and educational care plans for students who need
special nursing interventions during the school day.
-
Participates in the Response to
Intervention Team (RTI) as warranted.
Professional
Development
-
Utilizes continuing education
opportunities to enhance professional nursing
knowledge.
-
Participates in CPS and OSES
professional development opportunities.
-
Completes 80 hours of CEUs to
renew Type 73 School Nurse certificate every 4 years.
PHYSICAL REQUIREMENTS
Light Work: exerting up to twenty
(20) pounds of force occasionally, or up to ten (10) pounds of
force frequently, or negligible amount of force frequently to
move objects.
Even though weight of force moved may be only a negligible
amount, a position should be rated as light work (1) when it
requires walking or standing to a significant degree or
(2) when it requires sitting
most of the time but entails pushing or pulling of arm
control.
WORKPLACE ATTIRE (See section
4, Personnel Procedures-Professional Attire)
The CSN should wear casual
business attire that projects an image of professionalism and
is in accordance with school administrator's policies.
EVALUATION
Performance of the duties,
roles, and responsibilities of the Certified School Nurse
will be evaluated in accordance with provisions of the Board
of Education policy and contractual requirements for
Certified Personnel.
