  Description:
This position is responsible for the transitional components of the IEP for the 9th through 12 plans, administering career and vocational assessments when appropriate, tracking students for post secondary reporting including indicators. 

Qualifications:
Valid Illinois Special (Type 10) or Secondary (Type 09) Teaching Certificate
(LBSI) Endorsements
3 to 5 years of High School Special Education related experience
Experience with the following: 
      summary of performance
      IEPs especially the career components of 9th - 12th graders,
      transition planning,
      indicators 13 & 14 
      working with cognifively impaired students, and
       career inventories and assessments
      College, Career and Vocational planning
Provocational Experience a plus
Bilingual (English and Spanish) a plus
 
Responsibilities include but are not limited to:
Provide instruction and support per Individual Educational Plans (IEPs)
Serve as liaison to Case Manager with regards to transition planning and transition plans
Complete all required paperwork including quarterly IEP updates
Interface with the Special Education District of Lake County (SEDOL)  
Serve as a liaison to the middle school case managers for transition planning and development
